When it comes to credentialing, we’ll let you know exactly which professional licenses, personal documents, and other credentials you will need for each job. We recommend keeping the following documents and paperwork in an easily-accessible folder on your computer or phone.
Professional Licenses Needed
- Copy of State professional licenses (RN and CRNA)
- NBCRNA board certificate card
- Current AHA-approved ACLS, BLS and PALS if applicable
- Diplomas (BSN, Masters, DNP)
- Copy of updated CV
- Email addresses of 3 peer colleagues for professional references
- CEUs for the past 2 years
Personal Documents Needed
- Current mailing address
- Copy of Social Security card
- Color photo of Driver’s License
- DOB & Place of Birth
Insurance, Records & More
- Current flu shot record
- Annual TB record
- COVID-19 vaccination card
- Malpractice insurance
- Statements or claims report/actions taken against your license
How to Submit
We recommend using the app CamScan or similiar app. This allows you to take pictures on your phone and save them as PDF documents. Then, you’ll be able to email them to us or upload to your portal.
